  • Our Mission: To help our clients and their families to access, plan and implement health care services and achieve the best outcomes in the most respectful, compassionate and cost effective manner.
     
  • Assessment: Our Case Manager makes an onsite assessment of the client’s physical, mental, medical and social needs and then recommends the most suitable services and level of care the client will need. The assessment may continue periodically as needs of the client change.
     
  • Planning: Our Case Manager supplies a detailed rehabilitation plan or long term treatment plan. The plan is a collaboration of the client and families’ wishes after consideration of the client’s physical, mental and medical needs.  Client dignity, cost and entitlement eligibility are of prime consideration. Our Case Manager then recommends qualified local providers or research out of state providers if that is what is deemed appropriate.
     
  • Implementation: Our Case Manager will assist the family in implementation of the rehabilitation or long term treatment plan, providing expertise on local care givers for home care, assisted living facilities as well as long term and short term care options. The case manager can tour facilities, with or without the family, and make specific recommendations. Thereafter the case manager can monitor the client’s progress and insure that the contracted care is being delivered. The case manager will oversee the client’s care for families that live out of the area.
     
  • Coordination & Negotiation of Rates: Our Case Manager will coordinate eligibility with medicaid/medicare and insurance. For care that is not covered, the case manager can negotiate discounts with local providers on behalf of the client to insure the most cost effective solution.
     
  • Mediation: Navigating the complicated eligibility requirements and locating the most suitable health care services can be confusing and distressing to family members. Our case manager works with the family to provide clear guidance and expertise in providing care giving solutions for the elderly.
     
  • Special needs: Our Case Manager can attend physician appointment with the client and report to family members what care and follow-up is needed and arrange for this as requested.
